Comprehensive Support Services at Our HIV Clinic

Transportation Services
Access to transportation is vital for maintaining continuity of care. At Grace, we understand the importance of transportation, which is why we offer support through bus passes and Uber Health rides. Please note that consistent attendance is crucial to receive ongoing transportation assistance via Uber Health rides.
Support Groups
Finding a supportive community can make all the difference in your journey. That's why we facilitate two dedicated support groups: Sisters in Motion (meeting second Wednesdays of every month from 12-2p) and the Men’s group (meeting second Tuesdays of every month from 12-2p). These gatherings provide a safe space for sharing experiences, receiving support, and fostering connections. Plus, we offer meals and transportation to ensure accessibility for all.
Patient Incentives
When available, Grace Specialty Care offers incentives to support food, and hygiene security. We provide $25 Meijer Food Only gift cards to patients attending RW medical visits, and $15 Dollar Tree cards are offered when attending laboratory appointments. While we would like to offer these incentives when our patients are simply experiencing food or hygiene insecurities, we are required to demonstrate treatment adherence (attending medical or lab visits and improved health outcomes) when using grant funds to provide incentives.
Patient Advocacy
At our HIV clinic, Patient Advocates help with understanding the Ryan White continuum of care. This means they provide information and referral services to access health insurance, medication, Medical Case Management, and housing services through our community partnerships.
Mental Health
Comprehensive patient well-being is our priority. Individual screenings guide patient centered mental health care—therapy, psychiatric services, or a blend. Our trauma-informed approach ensures comprehensive outpatient psychiatric care, limited psychotherapy, and essential referrals.
Our W.O.R.K. Housing Program
Our WORK or Our Women Organizing Resource and Knowledge is a supportive housing project for women living with HIV (WLWH) with children. Our target population are WLWH between the ages of 18-30 with up to two children under the age of four. Family units can stay with the program for up to three years. During this time, we support access to education, job training, child-care, and transportation services. This project is funded through our sister clinic at Tolan Park Medical Building, with Dr. Lauren Touleyrou as the Primary Investigator.
Referral Support Services
Our team of Patient Advocates is here to help you find community resources that include traditional governmental benefits through MDHS, as well as other supportive services from our Ryan White community partners. These community partners offer a number of Ryan White-eligible services such as Medical Case Management, Housing resources, transportation, food bank, nutrition services, and assistance with health insurance premiums, to name a few.
